| Accommodation: | PolyU's Student Halls of Residence (Hung Hom) Remarks: Students of the same gender will be assigned to the same room. No single rooms will be arranged. |

| Meals: | Meals are included for the 3-day programme |
| Attachment with Faculty/ School: |
Participants will be attached to one of the above Faculty/ School during the camp. |

| Certificate: | A certificate of attendance will be awarded upon completion of the camp (3 days/2 nights). |

- Details of the payment procedure will be provided when the application is successful.
(Optional) School examination reports for the last TWO terms (final Term of 2021/2022 and 1st Term of 2022/2023) (You are recommended to submit it for our consideration, which will increase your chance of being admitted to the residential camp).
