All doctors in UHS are enrolled in the Government Vaccination Subsidy Schemes. Persons entitled to the government vaccination subsidy can receive vaccinations in our clinic without additional charge.

Preparation for your vaccination appointment

Depending on the type of vaccines, you maybe arranged to see our doctors for consultation first.
You will be asked to sign the vaccination consent form.
Please inform our doctors or staff about any previous history of vaccine or drug allergy.
Please bring along your vaccination record.
Remember to keep your vaccination record properly.
