Preparation for Your Vaccination

  1. Depending on the type of vaccines, you maybe arranged to see our doctors for consultation first.
  2. You will be asked to sign the vaccination consent form.
  3. To inform our doctors or staff about any previous history of vaccine or drug allergy.
  4. Please bring along your vaccination record.
  5. Remember to keep your vaccination record properly.

PolyU full time students and staff are eligible for our services.
One-month service grace period will be given to students upon confirmation of their eligibility for graduation.
