
The health and social care needs of our communities are growing more complex, especially among middle-aged and older adults facing chronic conditions, psychosocial distress, or functional decline. At the Joint Research Centre for Primary Health Care (JRCPHC), we believe that impactful solutions must be co-created—not imposed.
Rooted in our mission to integrate research, practice, and stakeholder engagement, JRCPHC is redefining how academic institutions work with communities. We are not only a research centre, but a catalyst for change—bridging the health-social divide through nurse-led models, participatory design, and strategic partnerships with NGOs and service providers.
Our focus is clear: to co-design scalable, community-based primary health care innovations that are grounded in the lived experiences of those we serve. We embed care where people live—not just where health services are traditionally provided—ensuring inclusivity, equity, and responsiveness. Through our strategic pillars of Health-Social Partnership, Digital Health Advancement, and Implementation Science, we aspire to influence local policies and set benchmarks across Asia.
