The aim of this application form is to apply or renew for card access to the SLS lab of the Department of Chinese and Bilingual Studies. The personal data are collected for security purposes, and for approving access to the lab.
The personal data collected in this form will serve as a record of the internal and external users of the lab. We shall not provide the information to non-PolyU parties, and for any unrelated purposes.
