Application for Record of IC Training

Important Note

a) Only applicants who SUCCESSFULLY COMPLETED ALL IC Study will be issued with the Record of IC Training.

b) Each application is applicable for one Programme of Study only. Please raise separate applications if you have more than one Programme of Study.

c) The maximum application period is 8 years from your admitted year. For example, an applicant who was admitted in Academic Year 2022/23 can apply for the Record of IC Training by the end of Academic Year 2029/30 at the latest.

d) IC will only proceed with your application when ALL necessary documents are submitted.

e) IC will withdraw your application if you are ineligible for the application and/or your personal particulars stated were inconsistent.


Application Procedures

Step 1: Transcript of Studies / Print-screen of Subject Results (PDF Format)

Students may either prepare an electronic copy of Transcript of Studies or print-screen your subject results which showing all subject grades along with your Student Name and Student ID in PDF format (file size under 10240 KB each, maximum of 5 files) for uploading.

Step 2: Application Form

Fill in this application form and successfully submit it with necessary documents. Your personal particulars stated in the form MUST be consistent to your registered Student Information and Hong Kong Identity Card.

Step 3: Payment

You will be directed to PolyU Finance Office "PolyU Online Payment Portal" for payment (Step 4). Please ignore it if you choose to pay in cash at IC Student Enquiry Counter in person.  

Step 4: Application Fee (HKD $50)

You may either

a) pay online through PolyU Finance Office "PolyU Online Payment Portal". Please select

Please capture your acknowledgement receipt (in jpeg format, file size under 10240 KB) and submit it in step 5; or 

b) pay in cash at IC Student Enquiry Counter in person. Please inform our staff your application number when submitting the payment.

Step 5: Application Number

You will receive a computer-generated email with your application number after successfully submitting your application. This application number will be referred during your payment in IC Student Enquiry Counter, submitting payment receipt and/or any outstanding documents etc. It is a reference code of your application and is necessary for you to write down for reference later.

Step 6: Payment Receipt

Please submit your payment receipt with the application number (Step 5) in platform of Payment Receipt & Supplementary Documents Submission for Record of IC Training . No receipt is required if you pay in cash in person.

Step 7: Supplementary Documents

In case you have outstanding or supplementary documents to be submitted, please submit them via platform  Payment Receipt & Supplementary Documents Submission for Record of IC Training  with the application number (Step 5) produced in your submitted application. 

Step 8: Notification Email and Collection of Record

A notification email will be sent to your registered mailbox for collection of Record of IC Training. Please bring along your Student ID Card / HK Identity Card for vertification.
