Each service unit will receive:

• Two Detachable Laser Indicators
• Instruction manual

• Attach the "Detachable Laser Indicator" to the user's mobility aid, such as a cane or walker, to allow the user to conduct walking training.

1. Complete and submit the online application form.

2. Selected organizations will receive a notification email of successful application.

• If many organizations meet the trial activity requirements, the project team will select the most suitable organizations to participate in the trial first.
• Organizations not selected will automatically be included in the next round of selection and do not need to reapply.
• The trial activity will continue to be implemented; organizations that have not received a successful application notification, please wait patiently.

• Each application can only cover one designated elderly care unit/organization.
• Applications with incomplete information or insufficient required details will not be processed.
• Please provide all application details in the form to facilitate follow-up by the project team.
• The information provided in the application form will determine your organization's priority for the trial activity.
• If additional information is needed, the "Jockey Club Smart Age Hub" project team will contact your organization's representative.
• All applications must be submitted via the online application form; applications submitted by other means will not be accepted.
• The project team reserves the final right to decide on participation in this trial activity.
