Referral Letter

According to the law in Hong Kong, service user who receives physiotherapy or occupational therapy has first to obtain a referral from a doctor (General practitioner / specialist), a dentist or a veterinary surgeon.

A valid medical referral with the signature and chop of doctor has to be issued within 6 months. The Clinic reserves the right to decline the service delivery to any service user who is not able to present a valid medical referral.

Appointment Booking

Physiotherapy service (based on treatment of single body part) is 30-45 minutes per session. Please inform the clinic receptionist with your conditions / issues (as stated on the medical referral) when booking the appointment(s) for our services.

If you have conditions with more than one body part, please inform the clinic receptionist when appointment booking so that sufficient time can be allocated for your treatment. Consultation fee will be charged based on the time duration of the treatment session.

Occupational therapy service is 45 minutes per session.

Attending Your First Appointment

Service user should arrive 15 minutes earlier with the following documents for registration at clinic reception:

  1. HKID card and a valid medical referral
  2. AIA HealthCard and PolyU Staff ID card (For University staff and dependant)
  3. PolyU Student ID card (For PolyU / CPCE student)

Subsequent Appointments

It is recommended that service user should book 4 subsequent appointments (in advance) to ensure that the rehabilitation sessions are implemented and scheduled in a timely manner and that no delays occur due to incompatible schedules.

Advance booking of appointment one month ahead is available on:

  1. Occupational Therapy: 1st of the present month
  2. Physiotherapy: 10th of the present month

Request for Occupational Therapy Written Report

Service user who would like to request for occupational therapy written report can approach the clinic receptionist for report arrangement and payment.

Service user who has terminated the service for more than 3 months needs to have a consultation with the occupational therapist of the Clinic prior to the delivery of the report.

Special Attention

  1. What to Wear: Please wear or bring suitable exercise clothing for each visit.
  2. For service users having the following implants, please inform the clinic staff before entering the Clinic’s treatment area.

Payment

Payments in cash, EPS, VISA, Mastercard and UnionPay are accepted.

For AIA HealthCard holder, service charge has to be settled with presentation of the AIA HealthCard. (Service user who cannot produce the Card has to settle the account in cash, EPS or credit card. The receipt will be offered for service user to reimburse the insurance claim with AIA afterwards.)

Late, No Show and Short Cancellation Policy

For service user who has made a prior appointment with the Clinic but cannot attend, please cancel the appointment at least 24 hours in advance.

For service user who does not attend the appointment (no show), the Clinic reserves the right to charge fully for the default visit. This will also apply to same-day cancellations.

For late arrival, the treatment session with our therapists will be shortened. There will not be refund for the forfeited treatment time. If being late for 15 minutes or more, your appointment will be cancelled and you may need to reschedule a new one.
