To help us have more ideas about your talent, Please feel free to share any video clips of your performance/ talent via providing a shared link of YouTube/ SharePoint/ Google Drive etc. You may also choose to send the shared link to polyu.talents@polyu.edu.hk within 3 days after submitting the Membership Registration.

To ensure you are well informed of the upcoming events from PolyU’s Got Talent’s team under the Student Affairs Office (SAO) may use your name, phone number, email address and correspondence address to send you relevant information through various communication channels. We need your consent before this service can be extended to you.

Statement and Declaration on the collection of personal data

All information given in this form will be treated in STRICT CONFIDENCE.

The personal data in relation to your application will be used by SAO to assess your suitability for the application that you are applying for. It may be provided to authorised parties to process the information for purposes relating to the application where applicable.

It is our policy to retain the personal data of unsuccessful applicants for future reference purpose for a period of no longer than four years.

Provision of full and complete information in your application and completion of all items on the application form is obligatory for selection purposes. Failure to provide these data may affect the processing and outcome of your application.

Under the Personal Data (Privacy) Ordinance, you may request for access to, and/or correction of your personal data in relation to your application. If you wish to do so, please write to polyu.talents@polyu.edu.hk  (Attn: Student Development Section).

I declare that the information given by me in this application form is complete and true to the best of my knowledge and belief. I authorize the Student Affairs Office (SAO) to handle and use the personal data/information provided by me and any supplementary information as required by SAO for activities relating to the handling, assessment of this application. I understand that the above personal data/information will be kept for a period of no more than four years under normal circumstances, subject to the full erasure within one year upon my graduation/departure from the PolyU.

I also authorize SAO to disclose the personal data provided to those relevant parties: for the above-mentioned purposes; where I have given my consent to such disclosure; and where such disclosure is authorized or required by law.
