This form is made reference to the “New Employee Safety Induction List” of Health, Safety and Environment Office (HSEO) of The Hong Kong Polytechnic University, version 2.1 dated May 2014. It serves to fulfil relevant part of the statutory requirements stated in section 6, 2(c) of the Occupational Safety & Health Ordinance for provision of information, instruction, training and supervision to ensure, so far as reasonably practicable, the health and safety of employees at work. Induction records are kept by Departmental Health and Safety Officer (DHSO) of Academic Registry (AR).
