Step 1: An email from IC General Office (ICGO) will be sent to your registered mailbox to inform you of your eligibility. You may contact ICGO for any queries regarding your eligibility.
Step 2: Complete and submit this online application form with your recent photo (in jpeg format, file size under 10240 KB). Please include your home institution student number in your JPEG filename. If any personal particulars stated in your application form are inconsistent, your application will be withdrawn and the application fee will NOT be refunded. You will need to submit another new application with application fee for the Certificate.
Step 3: Once you submit the application form, you will be directed to PolyU Finance Office "On-line Card Payment System" for online payment with credit card. Please ignore it if you have decided to pay in cash at IC Student Enquiry Counter in person.
Step 4: A computer-generated email with your application number will be sent to your registered mailbox once you have successfully submitted your application. This application number is a key number for our application references, please record it for later use.
Step 5: You may pay the application fee of HKD40 (non-refundable) by the following methods,
a) Pay via PolyU Finance Office "On-line Card Payment System" with credit card. Please select

You are required to capture your acknowledgement receipt (in jpeg format, file size under 10240 KB) and submit it in Step 6; or
b) Pay in cash at the IC Student Enquiry Counter in person. Please inform our staff of your application number (mentioned in Step 4) when settling the payment.
Step 6: The payment receipt obtained via On-line Card Payment System should be submitted to the platform "Payment Receipt and Supplementary Documents Submission for Certificate Application on MBST (IC-T16)" with the application number produced in your submitted application (mentioned in Step 4);
**No receipt is required to submit to the Payment Receipt and Supplementary Documents Submission for MBST (IC-T16) and Confined Space Operation (IC-T46) Certificate Application platform if you pay in cash at the IC Student Enquiry Counter in person.
Step 7: Submit supplementary document(s) in the platform "Payment Receipt and Supplementary Documents Submission for Certificate Application on MBST (IC-T16)" with the application number produced in your submitted application (mentioned in Step 4);
Step 8: An email notification will be sent to your registered mailbox for collection of the MBST certificate once it is ready. Please bring your HK Identity Card and Student ID Card along to the IC Student Enquiry Counter for verification.
