PolyU Form Service (PFS)

PolyU Form Service (PFS) is an easy-to-use, full-featured, web-based tool for departmental administrators to create and manage e-forms, for collecting essential information from targeted user group(s) in support of administration activities (e.g. online registration forms for workshops, conferences and other types of events)

PFS Feature Highlights

Here are the special features offered by PFS:

  • 3 types of form available (refer to the table below)
  • Different form templates available
  • Integration with FO’s Online-Payment Gateway
  • Support of different target user group(s) for each form
  • Support the display of collected data graphically and statistically
  • Support the export of collected data (in Excel and SPSS format)

PFS supports the following 3 types of online form based on different access mechanisms:

Type Mechanism for Users to Access the Form
Authentication Form Targeted users have to authenticate with their NetID/ NetPassword for access
Form by Invitation (token) Targeted users have to present the token that has been sent to them for access
Open Form No authentication is needed

Differences between PFS & mySurvey

PFS is implemented with the same underlying software used for building the popular mySurvey service. Although their User Interface look very similar, they are intended to support different business activities of different users:

Business Activities Supported Research, Teaching and Learning Administration (including administration of Research, Teaching and Learning activities)
Eligible User Groups (Form Owners/ Administrators) Individual Current Staff and Students Staff members nominated by departments
