Webpage Development Services

This service provides free pre-tender website consultancy and web technology support.  In addition, the service also offers cost-recovery project management and professional standard compliant website development services to departments / offices.  

If you would like to engage the service, please submit your request via HOTS.

PolyU Form Service

This service provides an easy-to-use, full featured web-based tool for registered Departmental Form Administrators to create and manage e-forms for collecting essential information from targeted user group(s) in support ofadministration activities (e.g. online registration forms for workshops, conferences, and other types of events).

For general staff members, you may ask the Departmental Form Administrator to create and manage a PolyU Form.

For Departmental Form Administrator nomination, the application should be submitted at here.

Service Requests and Change Requests


Service level

All service request are classified as level 1 to 4 depending on its impact & urgency.

  • Level 1 – Service Request Resolution Time: 2 days
  • Level 2 – Service Request Resolution Time: 4 days
  • Level 3 – Service Request Resolution Time: 6 days
  • Level 4 – Service Request Resolution Time: N/A

All Incident are classified as level 0 to 4 depending on its impact & urgency.

  • Level 0 – Incident Resolution Time: 4 hours
  • Level 1 – Incident Resolution Time: 1 day
  • Level 2 – Incident Resolution Time: 2 days
  • Level 3 – Incident Resolution Time: 5 days
  • Level 4 – Incident Resolution Time: 10 days
