When filling the on-line application form, please note that under “Personal Particulars”: The “Chinese Character Code” is for the HK ID card holders. Non-local applicants may ignore it. Applicants from the Chinese Mainland who do not possess a passport can enter some random numbers for the field “HKID Card / Passport number”.

Under “Qualifications”, please input as many details as possible. Academic qualifications such as transcripts, SOA exam results, TOEFL score, etc, should be scanned and uploaded with the application.

After finishing all the on-line application procedures, an application number shall be given to each applicant. Applicants should then pay the application fee. However, the university does not accept credit card payment for application fees, even for local Hong Kong students. Non-local applicants should send a Bank Draft or Money Order of the required amount (e.g. from Bank of China) to

Applicants should quote their application number at the back of the Bank Draft or Money Order (as a remark). The Bank Draft or Money Order should be made payable to “The Hong Kong Polytechnic University”. The admission process would be nullified if the application fee is not received by a certain date.

內 地 申 請 者 ﹐除 在 線 申 請 (on-line application) 外 ﹐所 需 提 交 至 本 系 (香 港 理 工 大 學 應 用 數 學 系) 的 文 件 如 下:

  1. 填 妥 之 “ 香 港 簽 証 / 進 入 許 可 ” 申 請 表 格 ID(C)936
  2. 履 歷 [Curriculum Vitae]
  3. 有 院 校 印 鑒 證 明 的 學 業 成 績 表 (英 文) 正 本 [Certified Copy of Applicant's Academic Transcript]
  4. 身 份 證 副 本
  5. 常 住 人 口 登 記 表 副 本
  6. 財 政 狀 況 之 證 明 [Proof of Applicant’s financial support] (e.g. a bank statement showing an amount equivalent to one year tuition fee + accommodation fee + living expenses)

學 生 一 經 被取 錄， 本 系 將 隨 即 替 其 向 香 港 入 境 事 務 處 遞 交 簽 證 申 請 及 其 他 所 需 文 件 ， 如 ID(C)936B 等 。 申 請 獲 批 後 ， 本 系 將 先 替 同 學 繳 交 港 幣 160 元 簽 證 費 ， 並 將 有 關 「 進 入 許 可 」 標 籤 寄 往 內 地 院 校 轉 交 學 生 。 學 生 可 憑 標 籤 直 接 向 其 戶 口 所 在 地 的 公 安 機 關 出 入 管 理 部 門 申 領 《 往 來 港 澳 通 行 證 》 （ 簡 稱 “ 因 私 通 行 證 ”）及 相 關 赴 港 簽 注 再 將「 進 入 許 可 」 標 籤 貼 於 申 領 到 的 “ 因 私 通 行 證 ” 空 白 簽 注 頁 內，便 可 來 港 就 讀 。 由 於 辦 理 通 行 證 需 時，學 生 收 到「進 入 許 可」標 籤 後，應 立 刻 申 請 “ 因 私 通 行 證 ”，並 於 抵 港 後 交 回 港 幣 160 元 之 簽 證 費 予 本 系。
注 意 : 根 據 香 港 入 境 條 例 , 所 有 學 生 簽 證 持 有 人 不 得 在 港 工 作 , 全 職 / 兼 職 / 實 習 均 屬 違 法 。學 生 來 港 後 應 即 時 (在 指 定 限 期 內) 到 教 務 處 (Academic Secretariat) 辦 理 入 學 註 冊 手 續 (Registration) 。屆 時 所 有 申 報 之 學 歷 證 明 文 件 正 本 將 由 教 務 處 核 實﹐及 學 生 須 即 時 繳 交 有 關 學 費。

申 請 學 分 轉 移 (Credit-transfer) 的 費 用 為 每 學 分 港 幣 145元。申 請 學 分 轉 移 的 學 生 必 須 於 申 請 時 繳 付 有 關 費 用。

內地學生須於到港後三個月之內到中央人民政府駐香港特別行政區聯絡辦公室〈中聯辦〉辦理『報到』的有關手續。

中聯辦只於星期二上午九時半至中午十二時及下午二時至四時半辦理以上提及之手續。
