disability are eligible to apply for campus parking
permits. There are, at present, designated parking
spaces for disabled persons |

Applicants are required to fill
in the application form obtainable from the general
office of his/her academic department or the
Facilities Management Office (FMO), at P110. Forms
FMO CS 21/22 can also be downloaded at
http://www.polyu.edu.hk/fmo/useful/download.html. |

Completed application form
should be endorsed by the Head of the department
concerned and returned to FMO together with the
following:
photocopy of vehicle license
photocopy of driving license
photocopy of student I.D. card
photocopy of certification of handicap issued by the
Government
photocopy of third party insurance policy. |
