form obtainable from the general office
of his/her academic department or the
Facilities Management Office (FMO),

form should be endorsed by the Head
of the department concerned and returned
to FMO together with the following:
photocopy of vehicle registration document (both sides)
photocopy of driving license
photocopy of student I.D. card
photocopy of certification of handicap
issued by the Government
photocopy of third party insurance
