Non-local Students under 18 year-old (minors)

Please note that all non-local minors should fill out a Departure Record Form and submit the duly filled form to ST314 at least 14 days before leaving Hong Kong.   Minors should also asked their parents to either send a written consent to SAO by email (sanonloc@inet.polyu.edu.hk) or by fax (2363 5815) as supporting to your application.  After your Departure Record Form is endorsed by your guardian, Mrs. Dorinda FUNG (Director of Student Affairs), we'll send you a scanned copy of the form for your reference.
