First-aid Box Management

FMO takes an initiative in compiling guidelines for University's departments / offices / centres / units in the management of first-aid boxes. The guidelines are listed hereunder:

(A) Supply of first-aid box

  1. For new provision of first-aid box:
    FMO will arrange to deliver and, if necessary, to install the first-aid box including all the required materials inside (Appendix A);
  2. For replacement of an existing first-aid box:
    FMO will arrange to deliver and, if necessary, to install the replacement first-aid box;
  3. Departments can send requests by forwarding the duly completed form - FMO TS17 (the form can be downloaded from http://www.polyu.edu.hk/fmo/download/fmots17.pdf).

(B) Supply of materials inside a first-aid box

  1. Ten items as prescribed by the relevant legislation will be supplied (please refer to Appendix A for details of the items);
  2. Departments can place order directly with the appointed supplier via fax by forwarding them the duly completed form - FMO TS18 (the form can be downloaded from http://www.polyu.edu.hk/fmo/download/fmots18.pdf) for normal refill of stipulated items;
  3. Upon receipt of the request, the supplier will deliver the required items within the first 3 working days in the ensuing month;
  4. Invoices will be sent directly to the department thereafter for payment settlement;
  5. Departments can purchase any of the items from the Convenience Store on campus or any medical store outside for ad hoc requirements.

In order to give a simple illustration on the above-mentioned arrangement, a flowchart is indicated in Appendix B.
